Program for Assertive Community Treatment (PACT)

Nurse

Full-time position available providing a range of nursing duties, including the development of medication policies and procedures, medication administration and monitoring, and consultation to clients, families and staff about medication/medical issues; other responsibilities include treatment planning, implementation and documentation, outreach, support, and case coordination as part of an interdisciplinary team.

Qualifications: BSN and at least 1 year experience treating persons with serious mental illness; substance disorder, psychosocial rehabilitation and assertive community treatment experience preferred. Valid MA driver's license required. Reliable transportation required.

Community Based Flexible Supports

Assistant Director

Full-time position available.  We are looking for an Assistant Director of CBFS Services, which offers high quality, person-centered rehabilitation services to individual diagnosed with chronic and persistent mental illnesses. Persons served may also have co-occurring diagnoses of PTSD or substance abuse. The Assistant Director will assist in overseeing the clinical and rehabilitative operations of CBFS Services. S/he will ensure services are person-centered, recovery-oriented and inclusive of families and significant others. S/he will also ensure adherence to Edinburg Center/CBFS Services policies and procedures and compliance with all state and local licensing/certification requirements and regulations. 

 

Qualifications: Licensed Practitioner of the Healing Arts and 3 years post Master’s work experience required. Training and experience in Illness Management and Recovery (IMR), Motivational Interviewing and/or Cognitive Behavioral Therapy strongly preferred.  Supervisory experience required.  Experience working with adults with mental illness required. Valid driver’s license required.

Licensed Practical Nurse

Part-time position (32 hours) available in CBFS Services at a residential program providing medical oversight along with recovery-oriented psychosocial rehabilitation services, case coordination, advocacy, support, linkage, and outreach to adults with mental health issues living in the community.  Supervises staff as assigned.

Qualifications: LPN and at least two years of experience in the mental health field required. BA or BS preferred. Supervisory experience preferred.  Experience in relevant evidenced based practices preferred. Valid driver?s license required.  First Aid and CPR certification within three months of hire, as required.  Skilled in the use of Microsoft Word within three months of hire required. 

RESIDENTIAL RECOVERY SUPERVISOR

 Full-time position available in CBFS Services at our residential program in Woburn providing recovery-oriented psychosocial rehabilitation services, case coordination, advocacy, support, linkage, and outreach to adults with mental health issues living in the community.  Supervises staff as assigned.

 

Qualifications: BA or BS and at least two years of experience in the field required. Advanced degree or matriculation in an advanced degree program in the field preferred. Experience with Applied Behavior Analysis strongly preferred. Supervisory experience preferred.  Experience in relevant evidenced based practices preferred. Valid driver’s license required.  First Aid, CPR, and MAP certification within three months of hire required.  Skilled in the use of Microsoft Word within three months of hire required. 

Certified Nursing Assistant

Part time (30 hour) position available in our new CBFS program in Belmont providing basic care services such as assistance with bathing and grooming, monitoring medical equipment, and checking vital signs. Also work as part of a team to provide recovery-oriented psychosocial rehabilitation services, case coordination, advocacy, support, and to adults with mental health issues living in a CBFS residential treatment program.

Qualifications:  High school diploma or equivalent required.  Certification and experience as a Nursing Assistant required.  Experience in the field of mental health preferred. Valid driver’s license required.  First Aid, CPR, and MAP certification within three months of hire required.  Skilled in the use of Microsoft Word within three months of hire required.

Recovery Coordinator

3/4-time positions (30 hours) available in CBFS Services at 169 Elm Street providing recovery-oriented psychosocial rehabilitation services, case coordination, advocacy, support, linkage, and outreach to adults with mental health issues living in the community.

Qualifications: BA or BS and at least one year of experience in the field required, two years in the field preferred. Valid driver’s license and reliable means of transportation required. First Aid, CPR, and MAP certification within three months of hire required. Skilled in the use of Microsoft Word within three months ofhire required.

Intensive Community Supports

Case Coordinator

Part-time (24 hour) position available in an innovative, community-based, step-down program to assist mental health consumers to avoid unnecessary hospitalization or transition successfully from the hospital to community living. Responsibilities include: case coordination, support, transportation, skills training, linkage and outreach as a member of an interdisciplinary team that utilizes an assertive outreach model to promote a culture in which each consumer’s subjective experiences, points of view and preferences are recognized, respected and integrated into all treatment, rehabilitation and support services. Participates in all program planning processes and provides direct services that promote consumer recovery, self-determination and decision-making.

Qualifications: BA in MH related field; at least 2 years experience with adult mental health population; experience in assertive community treatment (ACT) model programs and knowledge of psychosocial rehabilitation preferred. Valid driver’s license required. he ICS Team is an interdisciplinary team that utilizes an assertive outreach model to promote a culture in which each consumer’s subjective experiences, points of view and preferences are recognized, respected and integrated into all treatment, rehabilitation and support services. Participates in all program planning processes and provides direct services that promote consumer recovery, self-determination and decision-making

Respite Counselor II

Full and part-time positions available in an innovative, community-based, step-down program to assist mental health consumers to avoid unnecessary hospitalization or transition successfully from the hospital to community living. Primary responsibilities include providing direct client services in a 24-hour respite component.

RESPITE COUNSELOR

Part-time (20-hour) position available in an innovative, community-based, step-down program to assist mental health consumers to avoid unnecessary hospitalization or transition successfully from the hospital to community living. Primary responsibilities include providing direct client services in a 24-hour respite component.

Potter Place

Counselor I

Full-time position available in a community-based, clubhouse model program. Must have the ability to coordinate with a team of members and staff in all aspects of program functions. Responsibilities include instruction, guidance and support in prevocational and vocational needs of program participants.

Qualifications: High-School Diploma or equivalency required. Certificate in Human Services or related field preferred.  Experience with Certified Clubhouse program preferred.  Must have lived experience with a mental health condition.  Valid Massachusetts drivers license required.

In-Home Behavioral Services

In-Home Behavior Therapist

Fee-for-service position working with youth and their families to treat challenging behaviors that interfere with the youth’s successful functioning. Responsibilities include conducting functional behavioral assessments, development of individualized behavioral goals and plans, supervision of Behavior Monitor and coordination of treatment. Primarily late afternoon and evening hours.

Minimum Qualifications: MA in human service-related field with at least two years of experience conducting functional behavior assessments of youth with serious emotional and behavioral disturbance, designing / monitoring behavior plans based on the assessment, and training parents/caregivers to implement positive behavior plans based on principles of applied behavior analysis. Board Certified Behavior Analyst (BCBA) or enrolled in training program leading to certification preferred.
 

In-Home Therapist

Fee-for-service or part-time position providing strength-based in-home therapy to youth and family to treat the youth’s behavioral health needs and to improve the family’s ability to provide effective support for the youth’s successful functioning in the home and community.

Minimum Qualifications: Licensed Master’s-level Mental Health Clinician with at least two years experience working with youth and families. Experience with in-home therapy and applied behavior analysis preferred.

Therapeutic Mentor

Fee-for-service or part-time position providing one-to-one strength-based supports, coaching and training to youth to improve daily living, social and communication skills in the youth’s home or in community settings.

Minimum Qualifications: 21 years of age or older; Bachelor’s degree in a human service field and one year experience working with youth; OR Associate’s degree in a human services field and one year of experience working with youth; OR high school diploma or GED and a minimum of two years of experience working with youth

House Manager

A full time House Manager position is available for a residential program in Bedford. Provide direct care and administrative coverage for a residential program serving adults with developmental disabilities and progressive medical needs. Responsible for the daily clinical and administrative functions of the program including the monitoring of ISP’s, supervision of staff, and scheduling coverage.  The House Manager will be responsible for the coordination of all medical appointments. Ensure the individuals stability, safety, health promotion and maintenance, as well as acquisition of community living skills. Accompany clients on appointments and community outings. Work collaboratively with other treatment providers, family, and guardians to maximize each person’s dignity and quality of life. Assist in all aspects of home life such as meal planning and preparation, budgeting and leisure time.
 

Qualifications: Experience working with adults with developmental disabilities and progressive medical needs preferred. BA in related field preferred. Valid driver's license is required.  First Aid, CPR and MAP certification within three months of hire required.
